Registration and Consent
Please complete the WAIVER OF CONSENT, LIABILITY AND INDEMNIFICATION AGREEMENT below. Sections marked with (*) must be provided.
Consent, Waiver of Liability and Indemnification Agreement
I am a participant in a consultation/session (“session”), workshop or retreat offered by Therese Pautz and Boldly Radiant® (“Boldly Radiant®”) and understand I may receive information and instruction about yoga, Ayurveda, marma points, mudra, diet and exercise.
I recognize that parts of the session, workshop or retreat may require potentially strenuous physical and mental exertion. I am voluntarily participating in these activities with full knowledge of the risks involved and hereby assume all risks related to my participation, which may include physical, emotional and mental traumas, distress or injuries.
It is understood and agreed that the session, workshop or retreat is not meant to diagnose, treat or remedy any illness, disease, injury, physical condition or mental disorder.
I understand that it is my responsibility to consult a physician prior to and regarding my participation in a session, workshop, or retreat. If I have consulted a physician, I have taken the physician’s advice. I will abstain from activities that are contraindicated by my medical or emotional condition. The responsibility for following the limitations advised by my doctor is solely mine. I will not engage in any activities which I feel may be inadvisable based on my physical/medical history or personal judgment.
I represent, declare and warrant myself physically and mentally sound and capable of participation. I further represent that I do not have any physical or mental condition that would prevent my full participation or expose me to any risk of trauma or injury.
I understand and am fully aware of the risks and hazards involved. In consideration of being permitted to participate in the session, workshop or retreat, I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I might incur because of my participation in the session, workshop or retreat.
I agree to keep confidential any personal information that may be shared within the session, workshop or retreat context in whole group or private discussions. I agree not to share any information, verbally or in any other media form without express permission from the person(s) involved. In addition, this may include posts or sharing any photos, videos, recording or other media material of others without their express permission.
I understand that I might be photographed during a session, workshop or retreat activities and consent to allow my photo, video or likeness to be used for legitimate purposes by Boldly Radiant®.
I understand that I have been entrusted not to reprint, reproduce, copy, or share any material I receive without the written agreement from Boldly Radiant®. If I have specific questions about the use of any materials, I will direct it to a representative of Boldly Radiant®.
Except in cases of emergency, I agree to pay for any individual session or workshop which is not canceled at least 24 hours in advance. I recognize participation in retreats include additional terms and conditions outlined in a separate agreement.
I understand that session packages are non-refundable and non-transferable unless otherwise stated.
I release and agree to indemnify and hold harmless Boldly Radiant® against any and all claims, suits or actions of any kind whatsoever for liability, damages, compensation or otherwise brought by me or anyone on my behalf, including attorney’s fees and any related costs. If litigation arises pursuant to any claims made by Boldly Radiant® or anyone acting on its behalf, I agree to reimburse Boldly Radiant®.
If any portion of this agreement is unenforceable, the remaining portions shall remain in full force and effect. This agreement is governed by the laws of Minnesota.
BY SIGNING BELOW, I acknowledge that I have fully read, understand and consent to this agreement. I accept and agree to the terms and provisions contained. I am aware that by signing this agreement, I am waiving certain legal rights I or my heirs, next of kin, executors, administrators and assigns may have against Boldly Radiant®.
